ACO (Accountable Care Organization)

Care Gap Management

ReportingMD streamlines the care gap management process. Leveraging our clinical and financial data warehouse, clients are able to quicky and easily identify those patients with open care gaps, refine that list to only include patients without an upcoming appointment and outreach to those patients in a timely manner to ensure accurate coding during the measurement period. 

Additionally for those patients who are scheduled to see a provider the TOM platform is able to create a day of service report to help inform providers of open care gaps.
